- 轉銜教育與服務是從職業教育和生涯教育，而逐漸發展成形的教育實務，其目的乃欲協助身心障礙學生從高中畢業後，可以順利銜接至升學高等教育、就業、接受職業訓練、接受成人教育，以及成人的獨立生活，亦即是學校教育必須以學生的下一階段目標來規劃現階段之課程與學習活動。
轉銜教育須從小開始，高中階段更是關鍵期，這是一個整合性的教育措施，透過轉銜計畫全方位的規劃每一位學生的轉銜需求，並且連結至下一階段的學校或社政機構的服務提供。高中階段轉銜之「實證本位教學」和「實證本位預測指標」都有助於學校和教育行政相關單位的實務運作，以利身心障礙學生可以在高中畢業後成功轉銜至人生的下一段旅程。
